Heritage and Lifetime memberships are available to individuals of Portuguese descent. Please complete the affidavit below to confirm your Portuguese heritage before proceeding to membership checkout.

I affirm that I am of Portuguese descent and that the information provided above is true and accurate to the best of my knowledge. I understand that this statement is being provided to qualify for a PortAZ Heritage or Lifetime Membership.
